Our staff is committed to upholding the ethical standards established by the Commission on Rehabilitation Counselor Certification, as well as applicable state and federal laws concerning personal information protection.
We collect information solely for the purpose of conducting a comprehensive vocational evaluation that addresses the referral questions provided by the referring party. Evaluation reports will be shared with the paying or referring party, typically vocational rehabilitation agencies, through a password-protected client case management system or via secure/encrypted email.
All information is securely stored in password-protected locations managed by the assessment production company. Reports generated from this information are also kept on a password-protected computer network system or in locked paper files when paper assessments are utilized.
During the evaluation process, Dogwood Disability Consulting will only reach out to you via e-mail, phone, or text for scheduling purposes, meeting reminders, to provide assessment or virtual meeting links, and to gather necessary information for the assessment. Once the assessment and report are finalized and submitted to the referring agency, we will not send any further information using your contact details.
LIMITATIONS: We maintain individual confidentiality, except in cases where there is a threat of harm to oneself or others, evidence of abuse or neglect, or in situations where the law mandates disclosure.
A Waiver of Confidentiality authorization form is available upon request for individuals aged 18 and older or those not under legal guardianship, should you wish to share or include information with another party.
