 Dogwood Disability Consulting will not disclose or sell your personal information, ever. 

Dogwood Disability Consulting staff are required to adhere to the ethical standards set forward by the Commission on Rehabilitation Counselor Certification and applicable state and federal laws. 

Information is collected for the express purpose of completing a thorough vocational evaluation which answers the referral questions submitted by the referring party. Evaluation reports will be released to the paying/referring party (typically vocational rehabilitation agencies), via the password protected client case management system maintained by the vocational rehabilitation agency or via secure/encrypted email. 

All information is stored in password protected locations hosted by the assessment production company.  Reports created using this information are stored on a password protected computer network system and/or locked paper files where paper assessments were used.  

Dogwood Disability Consulting will only contact you via e-mail, phone, or text during the process of an evaluation for the purpose of scheduling, meeting reminders, to provide assessment or virtual meeting links, and to collect information needed for the assessment. Once the assessment and report are completed/submitted to the referring agency, Dogwood Disability Consulting will not send out any further information using your contact information. 

LIMITATIONS: Individual's confidentiality will be maintained, except in instances in which there is threat of harm to self or others, evidence of abuse or neglect, or other instances in which the law requires disclosure. 

A Waiver of Confidentiality authorization form is available upon request for those 18 and older or who are not subject to legal guardianship, should you wish to include/share information with another individual.
